Life and Accidental Loss Insurance coverages, provided through MetLife, offer you and your family financial protection when you need it most — after a death or serious injury.

Eligibility

As a represented employee, you are generally eligible for coverage on your date of hire. Review the Life Insurance Plans Summary Plan Description (SPD) for details.

You may be able to cover yourself, your eligible spouse or domestic partner and/or your eligible children. Certain exclusions apply; these can be found in the SPD. You may need to enroll in (and purchase) some coverage, and some coverage may be provided automatically and at no cost.

Evidence of insurability (EOI) may be required for you or your dependents to receive or increase coverage. You’ll be notified if EOI is required.

Coverage Options

Automatic Coverage
Generally, active employees automatically receive Basic Life Insurance and Basic Accidental Death and Dismemberment (AD&D) Insurance at no cost.

Optional Coverage
You may also purchase optional life and AD&D insurance*:

  • Group Term Life Insurance (Supplemental Life) (for yourself);
  • Spouse Life Insurance;
  • Child Life Insurance;
  • Supplementary Accidental Loss Insurance;
  • Spouse Accidental Loss Insurance; and
  • Child Accidental Loss Insurance.

*These coverages are fully paid for by you. You may enroll in, drop, increase or decrease any of these optional coverages at any time during the year or during the annual open enrollment period, subject to applicable EOI requirements.

You may choose the amount of optional coverage you want for yourself or your eligible dependents. There may be limits on the amount of coverage you can choose. Your Beneficiaries
You’ll need to name one or more primary beneficiaries for your life and AD&D insurance coverages.

You can also name one or more contingent (secondary) beneficiaries, who’ll receive benefits if all primary beneficiaries die before you. You can change your beneficiary at any time by contacting the carrier. If you die and don’t have a beneficiary on file, or if your primary and contingent beneficiaries have died before you, the benefit will be paid according to the Plan’s rules.

Your Dependents’ Beneficiary
You’re the designated beneficiary for plans that cover your spouse and/or child:

  • Spouse Life Insurance;
  • Child Life Insurance;
  • Spouse Accidental Loss Insurance; and
  • Child Accidental Loss Insurance.

Life and Accidental Loss Insurance Claims
If you or your covered dependents are injured or die, both the carrier and the Nokia Benefits Resource Center will need to be notified.

Contact the carrier to learn more about the steps to take to claim benefits, including providing a certified copy of the death certificate. You or your beneficiary should submit a claim within 90 days after the death.

Costs

You automatically receive Basic Life Insurance and Basic AD&D Insurance coverages at no cost. Enrollment is not required.

You pay the full cost of any optional life and/or AD&D insurance coverage(s) you elect with after-tax deductions from your paycheck. For your specific costs, visit the YBR website or call the Nokia Benefits Resource Center.

How Coverage Affects Your Taxable Income
Imputed income may apply, depending upon the type of coverage and coverage amount you choose.
