Vision Plan

The Vision Plan, administered through EyeMed Vision Care, encourages good eye care by offering coverage for routine eye exams and prescription frames and lenses. The plan has a network of participating optometrists and vision care centers that provide vision care services at negotiated rates. You are eligible for coverage if you are enrolled in the Medical Plan.

Eligibility

In general, you’re eligible for coverage on the first day of the month in which you attain six months of net credited service. For example, if your six-month anniversary falls on March 16, 2013, then you are eligible for coverage on March 1, 2013.

Coverage Options

If you’re enrolled in the Medical Plan, you will have coverage under the Vision Plan. Your benefits depend on whether you use in-network or out-of-network providers.

You can seek vision care from any optometrist. However:

  • If you use in-network providers: You receive the highest level of benefits.
  • If you use out-of-network providers: You receive a lower level of benefits and pay for services in full at the time that you receive care. You will then need to file a claim with EyeMed Vision Care to be reimbursed.
