Update: Dependent Verification Extended Through May 30

Did you submit your Dependent Verification information? Due to the large volume of last minute responses that have been submitted to Budco, the third-party organization acting on Alcatel-Lucent’s behalf, the deadline to submit your Dependent Verification information is extended until Friday, May 30, 2008. There will be no additional extension of the deadline. Your dependents may lose health care coverage if you miss the May 30 deadline.

What You Need to Do

• Review Your Dependent Verification Package*
  Budco mailed Dependent Verification packages the week of February 11, 2008. The packages were sent to the homes of employees and retirees that had dependents enrolled under the Alcatel-Lucent health care plans as of Monday, January 14, 2008.
• Verify Your Dependents
  Follow the directions in the package to complete and mail the requested documentation and information by Friday, May 30, 2008.

Confirmation Process

After Budco has reviewed your documentation, you will receive a confirmation letter from them advising whether each of your dependents is eligible or incomplete. Beginning Tuesday, June 10, 2008, Budco will mail final disposition letters to employees and retirees who have:

• Dependents deemed eligible for coverage, but who have not yet received confirmation letters;
• Submitted incomplete or partial documentation;
• Voluntarily dropped ineligible dependents during the verification process; or
• Not responded to the Dependent Verification package they received.

Dependents not deemed eligible will be dropped from coverage effective Tuesday, July 1, 2008. These dependents will be offered coverage under the Consolidated Omnibus Budget Reconciliation Act (COBRA) and will be mailed COBRA enrollment packages in late June.

The Help Line is available Monday through Friday, from 8:00 a.m. to 5:00 p.m., Eastern Time (ET), and will remain operational until 5:00 p.m., Eastern Time, on Friday, May 30, 2008.

*Employees and retirees who enrolled dependents in coverage since January 14, 2008, and Genesys and Alcatel Vacuum Products, are not subject to the verification. Additionally, if you already submitted your verification, please disregard this notice.
